• CPT Codes: 97802, 97803. 
• Are there specific ICD-10 codes (diagnostic) covered? For example: Diabetes, eating disorders, obesity, etc. 
• Do you have preventative benefits under the Affordable Care Act (ACA)? These codes generally cover at 100% with unlimited visits. A common example code includes ICD-10 Code Z71.3. 
• Is coverage available for virtual appointments nutrition, including telehealth or virtual counseling? 
• Do you need to reach your deductible first? If so, what is your deductible? 
• How many visits for nutrition counseling are allowed under your nutrition counseling insurance? 
• Are there any copays or coinsurances?
